New Patients

New Patients need to arrive 15 minutes prior to your scheduled appointment time. You must have your Driver's License and all current insurance cards when you come in for your appointment. Medicare ONLY patients must pay their 20% and deductible (if applicable) at the time of the visit. Patients that are Self Pay have two options. You can pay in full with a 25% discount or $100 is due at the time of the visit with payment arrangements required at the time of the visit.

Insurance Plans We Are Contracted With

    Here is a list of insurances that we are contracted with as of 8/1/2010:
  • MEDICARE BCBS
  • AZ FOUNDATION AETNA
  • CIGNA UHC
  • TRICARE MULTIPLAN / PHCS
  • IPA HUMANA
  • CAP LONG TERM CARE
  • MERCY CARE / MERCY HEALTHCARE GROUP
  • UNITED HEALTHCARE
  • Any Medicare Advantage Plan that is a PFFS.
  • If an insurance plan is an HMO and not listed above, we need an authorization before a patient can be seen. We will see Bridgeway and Phx Health Plan patients for vein problems, but they must have an authorization from a primary care provider. If a patient is an established patient, or AHCCCS is secondary to Medicare, they may be seen in our office.

    Please note that Dr. Schilperoort works almost exclusively at Tri City Surgery Center for outpatient surgeries, although most vein procedures are performed in our office. Since Tri-City Surgery Center is not contracted with some of these plans, we check benefits before scheduling a patient to make sure an operation can be performed without penalizing the patient for out-of-network costs.

    Medicare, TriCare, Blue Cross, United Healthcare and the AHCCCS plans are not a problem at Tri-City Surgery Center, as they do have contracts.

HMO PLans

If you have an HMO Plan, we will accept them with an authorized referral from your PCP.
